Definitions

Account: A single logon to a system.

Administrator: A user or account that implements decisions made by a system owner which grants access to others, specifies the level of their access and revokes access or deletes the user

Communal Account: A logon used by multiple people.

Owner: The persons or entity that make decisions on how a system is used and who operates or uses a system.

System: A computing platform that can process, store, or transmit information.

User: A person who logs into a system.

What System, Software, Service or Subscription

What is the name of the system or software?

What is the URL or address used to access the system?

What name, postal and/or email addresses are used:

for the system registration?

to which notices are sent by the system or system provider?

to which invoices are sent?

Owners - Administrators - Users

Who is the owner of the system?

Does the system have an administrator(s)? If so, who is that person or those persons?

If there is an administrator, is it an account specific to an individual or a single communal account accessible by more than one person?

If an account specific to an individual, who is the person with that access?

If a single communal account, what is the name of that account and who are the people with the password for that account?

Does the system require a level of complexity for a password for an account, such as the number and type of characters? If so, what are the password requirements?

Does the system have a password policy such as requiring periodic password changes, not allowing passwords to be reused, requiring multi factor authentication, etc.?

Does the system, or can the administrator of the system, require a user to change their password either periodically or 'on demand'?

Does the system or software allow for differing levels of access?

For each level of access to the system, if any, identify the user and level of access.
